Toxic Encephalopathy, or "Cranium Cracking Brain Pain" is definitely one of the worst. But I think I have to give the top slot to the "Ice Pick, Front and Back." It feels as though someone is stabbing you with an icepick in the sternum and spine and leveraging them apart. Its a lymph and porphyria issue caused by glutathione deficiency.
