Is exercise safe for someone with Fabry disease?

Yes, exercise is widely considered beneficial for managing the overall health of patients with Fabry disease. Research suggests that maintaining physical fitness can help mitigate some of the secondary effects of the condition, such as reduced exercise tolerance. However, because Fabry disease often impacts the heart (cardiomyopathy) and the autonomic nervous system—which regulates sweat production—you must be mindful of your body’s signals. Always seek medical clearance from your cardiologist or a specialist familiar with Fabry disease before beginning a new routine to ensure your heart function is stable for the intended level of exertion.

What types of exercise are recommended for Fabry disease?

The goal is to choose activities that improve strength and cardiovascular health without triggering overheating, which is a common trigger for pain crises in Fabry disease. Low-to-moderate intensity activities are generally the safest starting point. Recommended activities include:

  • Walking: A low-impact way to maintain cardiovascular health that can be easily adjusted based on how you feel.

  • Swimming or water aerobics: These are excellent choices because the water helps keep the body cool, preventing the overheating often associated with Fabry disease symptoms.

  • Yoga and Pilates: These focus on flexibility, core stability, and mindfulness, which can help manage the chronic pain associated with Fabry disease.

  • Light resistance training: Using light weights or resistance bands can help maintain muscle mass, provided it is done with controlled, steady movements.

How can I safely adapt my exercise routine on difficult days?

Living with Fabry disease means your energy levels may fluctuate significantly. On days when you experience increased pain or fatigue, it is essential to practice "pacing." Pacing involves breaking your activity into shorter, manageable segments rather than pushing through exhaustion. If you feel a pain crisis coming on or notice excessive breathlessness, stop immediately and rest in a cool environment. Listen to your body; if your resting heart rate is higher than usual or you feel lightheaded, it is a sign that your body needs a recovery day rather than a workout.

What is the role of physical therapy?

For many, a physical therapist (PT) is a vital member of the care team. A PT can create a tailored rehabilitation program that accounts for the specific musculoskeletal and neurological challenges of Fabry disease. They can help with gait training, balance, and specialized strength exercises that minimize strain on the heart. Since my Fabry Disease has caused hypohidrosis (I don't sweat enough) the best exercise I can think to get is to swim laps. My cardiologist says I have to exercise at least 3 times per week for at least 30 minutes since my heart is a use it or lose it muscle.

Be sure to exercise enough to get your heart rate elevated but not so much as to need medical assistance. Only swim with a lifeguard on duty and make others aware if you are experiencing any distress.
