Couple and Hidradenitis Suppurativa

Living with Hidradenitis Suppurativa can present unique challenges to romantic relationships and intimacy due to the physical pain, scarring, and potential for malodorous drainage associated with the condition. However, with open communication, mutual understanding, and proactive management, many individuals with Hidradenitis Suppurativa maintain deeply fulfilling and lasting partnerships.

How does Hidradenitis Suppurativa impact intimacy and relationships?

The physical manifestation of Hidradenitis Suppurativa—often involving painful abscesses and draining lesions in the groin, armpits, and under the breasts—can significantly impact body image and self-esteem. It is common for individuals to experience anxiety regarding how a partner might react to the appearance or the scent of the lesions. Because Hidradenitis Suppurativa is a chronic, often unpredictable condition, the physical pain and exhaustion from flares can also lead to a reduced libido or the need to avoid physical touch during active outbreaks. Acknowledging these feelings is a vital first step in preventing the isolation that this condition can sometimes foster.

What are effective communication strategies for discussing Hidradenitis Suppurativa?

Honesty is the foundation of navigating these challenges. When you feel ready, explain that you have a chronic inflammatory condition, not merely a skin issue, which requires ongoing management. Providing your partner with reliable information—perhaps by sharing resources from the 729 members of the DiseaseMaps.org community—can help demystify the condition. Establishing a "check-in" system where you can safely communicate your pain levels or flare-up status without fear of judgment can prevent resentment and foster a supportive environment.

How can couples maintain intimacy while managing the condition?

Maintaining intimacy when living with Hidradenitis Suppurativa requires creativity and flexibility. Consider the following strategies to preserve your connection:

  • Prioritize non-sexual intimacy: Physical closeness, such as holding hands, cuddling, or massage, can maintain a sense of connection even when sexual activity is painful or uncomfortable.

  • Modify sexual activity: Explore positions or activities that minimize friction or pressure on sensitive, affected areas.

  • Establish hygiene routines: Some couples find that having a post-intimacy hygiene routine helps alleviate anxiety regarding odor or leakage.

  • Focus on emotional safety: Create an environment where you feel secure enough to say "no" to intimacy without feeling guilty, ensuring that your partner understands this is about symptom management, not a lack of attraction.

What are the considerations for family planning and genetics?

Hidradenitis Suppurativa is not strictly Mendelian, but it does have a genetic component, with some studies suggesting a positive family history in 30% to 40% of cases. If you are considering starting a family, it is essential to consult with a clinical geneticist or a dermatologist experienced in reproductive health. They can provide a personalized risk assessment and discuss how your current treatment regimen—especially if using systemic immunomodulators—might need to be adjusted before conception.

When should couples seek professional support?

If the stress of managing Hidradenitis Suppurativa begins to cause persistent conflict, withdrawal, or significant changes in your emotional well-being, seeking a couples therapist who specializes in chronic illness is highly recommended. A therapist can provide a neutral space to process the grief associated with a chronic diagnosis and help both partners develop healthy coping mechanisms to prevent caregiver burnout.

It can be difficauky, but if the person loves you then they'll take all of you even the HS!!! Yeah it can be horrible and painful but they'll be supportive and helpful where they can.

It is always difficult to find a partner or maintain a relationship, regardless of the circumstances. No, I do not have a partner, but that is mostly by choice. I have read stories on Facebook where people have lost relationships because of HS, but some relationships have also been made stronger. It is always a good idea to be completely honest with someone with whom you are involved. If the person chooses not to be involved because of your illness, it is best for you. Nothing is worse than getting into a relationship with someone without compassion. Don't be discouraged by this: there are still compassionate people out there whose strength will strengthen you. You will find someone. Never give up hope.
