Is exercise safe for those with Castleman disease?

For most patients, physical activity is beneficial, but you must first obtain clearance from your hematologist or oncologist. Because Castleman disease can cause systemic inflammation and significant fatigue, the goal is to improve cardiovascular health and muscle tone without triggering "post-exertional malaise." Always prioritize consistency over intensity.

What types of exercise are recommended?

Low-impact activities that allow for easy adjustment of intensity are ideal for managing Castleman disease. Consider the following:

  • Walking: Gentle, short-duration walks help maintain mobility without overtaxing the immune system.

  • Yoga and Stretching: These promote flexibility and stress reduction, which is vital for patients dealing with the emotional weight of a chronic diagnosis.

  • Water Aerobics: The buoyancy of water reduces joint stress, making it an excellent option for those experiencing systemic inflammation.

  • Light Resistance Training: Using resistance bands can help combat muscle atrophy often seen during long treatment cycles for Castleman disease.

How should I pace my activities?

On "high-symptom" days, listen to your body and adopt a "pacing" strategy. If you feel increased fatigue or pain, reduce your activity level significantly or rest entirely. You should avoid high-intensity interval training (HIIT) or heavy contact sports, especially if you have enlarged lymph nodes or splenomegaly, as these conditions may require protecting your abdomen from physical trauma.

What is the role of physical therapy?

A physical therapist familiar with chronic illness can create a personalized plan for Castleman disease patients. They can assist in creating a gradual progression schedule, ensuring that your exercise program supports your recovery rather than hindering it.

Castlemans is so varied that one answer is no good. At one particular point I felt ok but had too much potassium in my boy so my heat was apparently really slow and they called me and told me don't eat nothing with potassium in it or stress out because I could have heart attack. Some can exercise some can't and some are unable to. Best talk to your Doctor.

Yes, to try and help build endurance and maybe help with the muscle and joint pain. I would ask your doctor before starting any exercise program. Low impact will probably be best.
