Is exercise safe for Medullary Sponge Kidney patients?

For the 347 members of our Medullary Sponge Kidney community, movement is a powerful tool for symptom management. Exercise helps improve hydration habits—a cornerstone of managing Medullary Sponge Kidney—and reduces the stress associated with chronic pain. Provided you are not currently experiencing an acute kidney stone episode or infection, staying active is highly recommended to prevent the deconditioning that often occurs with chronic illness.

Which activities are best for managing Medullary Sponge Kidney?

The goal is to choose activities that are sustainable and low-impact. We recommend the following to support your health while living with Medullary Sponge Kidney:

  • Walking or light jogging: Encourages steady hydration and keeps muscles engaged without excessive strain.

  • Swimming: An excellent, low-impact choice that allows for full-body movement without joint stress.

  • Yoga and Pilates: These help with core strength and stress reduction, which can indirectly help manage pain sensitivity.

  • Strength training: Using light weights or resistance bands helps maintain bone density and metabolism.

How can I safely start an exercise program?

Always seek medical clearance from your nephrologist before starting a new routine. When you begin, use the "start low, go slow" approach. If you experience flank pain or hematuria (blood in the urine), stop immediately and rest. On days when your Medullary Sponge Kidney symptoms are more pronounced, switch to gentle stretching or restorative movement rather than pushing through intensity.

The role of physical therapy and pacing

If you experience chronic back or flank pain associated with Medullary Sponge Kidney, a physical therapist can design a personalized program to strengthen the muscles surrounding the kidneys. Pacing is essential; listen to your body’s signals. If you have a high-pain day, prioritize rest and hydration over your usual workout intensity to avoid overexertion.

Next steps

  • Consult your nephrologist to ensure your specific anatomy is stable for exercise.

  • Keep a log of your physical activity and how your Medullary Sponge Kidney symptoms respond to different intensities.

  • Prioritize hydration before, during, and after every session.

Exercise should be done as tolerated. Some patients are in too much pain to move around as much as others but it is advised to do as much as possible within reason.

It depends on your pain and what you may tolerate. Me personally, I have many existing conditions that do not allow me to exercise.

I personally feel keeping your body in good shape and as healthy as your able to would greatly help you mentally and physically to better manage your pain and or while passing a stone work out to the best that you are able to but make sure you drink your Waight in waterso your body does not turn against you and cause you to become house ridden from dehydration because dehydration is your worst enemy And you will end up in the hospital before you even realize what’s going on plus the pain would be so bad you will wish to die... I ask the emergency doctors every time I end up in there if they would be willing to take the kidney out that bothers me the most as a joke that I wish someone would agree to do one day...
