Exercise is generally recommended for individuals with Nemaline Myopathy, provided it is approached with caution, medical supervision, and a focus on submaximal intensity. While Nemaline Myopathy involves muscle weakness and hypotonia, tailored physical activity can help maintain joint range of motion, improve respiratory function, and support overall quality of life. Is exercise safe for those with Nemaline Myopathy? Yes, but it must be highly individualized.

Because Nemaline Myopathy affects skeletal muscles, the primary goal of exercise is not hypertrophy (muscle building) but rather functional maintenance and preventing contractures. Overexertion can lead to muscle fatigue and potential injury, so activities must be performed at a low-to-moderate intensity that does not cause significant post-exercise soreness or weakness.

What types of activities are recommended for Nemaline Myopathy?

Low-impact activities that minimize gravity's strain on weak muscles are often best. For the 102 community members on DiseaseMaps.org living with Nemaline Myopathy, the following are often suggested:

  • Aquatic therapy/Swimming: Water provides buoyancy, allowing movement of limbs that might be difficult on land.

  • Gentle Yoga or Stretching: Focuses on maintaining flexibility and preventing the joint contractures common in Nemaline Myopathy.

  • Stationary Cycling: Provides controlled, repetitive movement to maintain range of motion.

  • Walking: If mobility allows, short, frequent walks help maintain bone density and cardiovascular health.

How can I start an exercise program safely?

Before beginning any new regimen, you must consult with a neurologist or a physical therapist who specializes in neuromuscular disorders. Start with a "pacing" strategy: perform short, 5-10 minute sessions and monitor your body’s response over the next 24 hours. If you experience excessive fatigue or muscle pain, reduce the intensity or duration. Always prioritize rest days to allow for recovery, as muscle fatigue in Nemaline Myopathy can be cumulative.

Why is professional rehabilitation important?

A physical therapist is essential for creating a safe, customized plan for Nemaline Myopathy patients. They can monitor for signs of overwork and ensure that the exercises specifically target areas of weakness without risking injury. Physical therapy also plays a critical role in respiratory health, teaching techniques to strengthen breathing muscles, which is vital for those with more severe forms of Nemaline Myopathy.

Next steps

  • Consult your neuromuscular specialist to receive clearance for a physical therapy evaluation.

  • Connect with others at DiseaseMaps.org to share experiences regarding which exercises have been most effective for your specific level of Nemaline Myopathy.

  • Request a referral to a physical therapist experienced in congenital myopathies.

Regular physiotherapy/hydrotherapy is recommended for people with this condition, as it is important to maintain muscle strength if possible. The physical activity should not be too intense however as this is difficult for those with muscle weakness.

Exercise is good but not to cause fatigue or break down muscles. Active life is good. Isometric exercise is best along with swimming/pool therapy!
